55 year old lady complained of discoloration of her both legs since 1 year. She gave history of cooking in front of an earthen oven with affected part being exposed to heat while cooking. Reticulate hyperpigmentation with erythema and atrophy was observed on the lateral aspect of the legs [Figure 1]. Erythema ab igne refers to reticulated erythema and hyperpigmentation often with epidermal atrophy, scaling and telangiectases resulting due to prolonged exposure to lower degrees of heat that is insufficient to cause thermal burn. This may arise from fireplaces, heating units, sauna-belts, laptops, or hot water bottles. [1] Patients suffering from cancer may develop erythema ab igne due to painful underlying lesion that were applied repeated local heat with water bottles. [2] It may also be seen in patients with hypothyroidism. Squamous and Merkel cell carcinomas may rarely appear from the thermal keratoses arising in erythema ab igne.
